The National Committee for Quality Assurance (NCQA) recognized our practice for providing highly coordinated care and long-term, participative relationships.

O’Kane. “The active, ongoing relationship between a patient and a physician in medical homes fosters an all-too-rare goal in care: staying healthy and preventing illness in the first place.”
Recognition as a PCMH shows that Union Medical Group has the tools, systems and resources to provide their patients with the right care at the right time and that they support the delivery of high-quality care.
A history of collaboration, since 2003
Union Medical Group is a private medical practice established in 2003 by Dr. Nereida Diaz-Johnson and Dr. Clark Philogene. The doctors have a close history of collaboration, having practiced together as staff physicians for the University of Medicine and Dentistry of New Jersey (UMDNJ) and, before that, as staff physicians for the Health Insurance Plan (HIP) of New Jersey.
Our doctors and clinical staff take pride in bringing excellent quality primary care to people from all cultural backgrounds. They provide patient-centered healthcare, particularly where language barriers have made obtaining such care difficult for members of minority communities.
Caring for the whole patient
Drs. Johnson and Philogene are internists – doctors of internal medicine – and are equipped to deal with whatever problem a patient brings, no matter how common, rare, simple or complex. Internists are specially trained to solve puzzling diagnostic problems and can handle chronic illnesses and situations where multiple illnesses may strike at the same time. They provide an understanding of disease prevention, mitigation and the meaning of healthy living. In addition to this care, they offer effective treatment of common problems of the eyes, ears, skin, nervous system and reproductive organs.
